I authorize JDRF to create a gift certificate on JDRF letterhead which will be accepted as valid gift certificate for guest's use at our business. Certificate will expire November 30, 2013. Business card must be submitted to JDRF.

To Qualify for Tax Receipt - You must submit an invoice to JDRF by December 1, 2012 stating NO CHARGE-GIFT IN KIND, showing fair market value and a detailed description of the items donated. “Services” and Gift Certificates are not eligible for receipts. Items valued over $500 must attach proof of value (ie. Receipt, brochure, or catalogue). All receipting must be approved by JDRF. Registered Charitable #11897 6604 RR0001


Approval: I, have read and accept the tax receipt conditions and authorize the donation as stated above.

Note: Due to Canada Customs & Revenue Agency (CCRA) regulations, JDRF is unable to provide a Charitable Tax receipt unless an official invoice or letter of request is provided for the in-kind donation.  All requests for art and jewellery valued over $500 require a written appraisal. You may choose to “write off” your gift as a business or marketing expense. Tax receipts will be issued prior to February 28, 2013 as long as above procedures are followed. In compliance with the Privacy Act, we cannot divulge buyer information.
